In Working Late, elite enemies are significantly more dangerous than standard hospital horrors. These special threats possess unique abilities that can instantly end your shift if you are unprepared. Unlike common enemies that patrol predictable routes, elite enemies often react to your actions, such as performing organ harvests or making too much noise. Understanding each elite type is crucial for surviving until 6 AM. This guide covers all known elite enemies, their spawn triggers, and proven strategies to counter them.
The Surgeon
The Surgeon is a tall, gaunt figure wearing a blood-stained lab coat. It is one of the first elite enemies you may encounter, typically appearing after you complete your first successful organ extraction. The Surgeon moves slowly but can teleport short distances to close gaps. Its primary attack is a bone-saw swipe that deals massive damage. It is highly sensitive to sound; running or dropping heavy equipment will attract it from several rooms away.
Strategy: Use slow, crouched movement when you suspect the Surgeon is nearby. Hide in lockers or under desks when it enters your room. It cannot open locker doors but will wait outside for a short time. If you need to move, wait for it to patrol to the far end of the hallway. The Surgeon is vulnerable to bright light; briefly flashing your surgical light in its face will stun it for 3 seconds, giving you time to escape.
The Administrator
The Administrator appears as a shadowy figure in a business suit, often seen reviewing charts in the main office. This elite enemy does not chase you directly. Instead, it manipulates the hospital environment. When the Administrator is active, doors may lock randomly, lights will flicker, and the ambient temperature drops. Its most dangerous ability is "Budget Cuts," which reduces the time you have to complete organ harvests by 15 seconds. The Administrator spawns after you have harvested three organs.
Strategy: The Administrator cannot be stunned or fought. You must complete your objectives faster. Prioritize finding organs in nearby rooms before it activates. Listen for the sound of shuffling papers; this indicates the Administrator is in an adjacent room. Avoid that area until it moves. Carrying a fully charged flashlight battery can reduce the duration of its "Budget Cuts" effect.
The Orderly
The Orderly is a massive, brutish enemy that patrols the basement and morgue areas. It is blind but has exceptional hearing. The Orderly will smash through barricades and doors if it detects you. Its attacks are slow but can kill you in two hits. It is immune to flashlight stuns. The Orderly spawns after a panic event, such as a failed surgery or a patient waking up. It will remain active for the rest of the night.
Strategy: Never run in the basement. The Orderly responds to vibration. Instead, walk or crawl through vents. You can distract the Orderly by throwing a surgical tool against a far wall. It will investigate the sound for 10 seconds. Use this window to cross open areas. The Orderly cannot fit through small gaps, so learn the crawlspace locations on the map.
The Patient Zero
Patient Zero is a unique elite enemy that only appears during the final hour (5:00 AM to 6:00 AM). It is a twisted, amalgamated creature formed from multiple failed organ transplants. Patient Zero is fast, aggressive, and can climb walls and ceilings. It will actively hunt you across the entire hospital floor. Its attacks inflict a "Corruption" status effect that slowly drains your health even after you escape.
Strategy: Save your strongest stun items for this encounter. Patient Zero is vulnerable to fire; using a flare gun or incendiary device will cause it to retreat temporarily. Focus on staying in well-lit rooms, as it hesitates to enter bright areas. You cannot outrun Patient Zero, so use the environment to break line of sight. Hide in rooms with only one entrance and wait for it to pass. If you have collected enough organs, the final extraction will trigger a cutscene that ends the encounter.
General Tips for Surviving Elite Enemies
Always listen for audio cues. Each elite enemy has a distinct sound: the Surgeon's saw humming, the Administrator's paper shuffling, the Orderly's heavy breathing, and Patient Zero's wet, slithering movement. Invest in the "Peripheral Vision" upgrade early, as it increases your awareness radius on the mini-map. Keep at least one stun item in your inventory at all times. Finally, learn the layout of the hospital. Knowing the shortest path between the operating room and the exit is your best defense against any elite enemy.
