Enemies are categorized into three primary groups: Roaming Entities, Room-Specific Threats, and Environmental Hazards. Roaming entities patrol the hospital corridors, room-specific threats appear in designated areas like the morgue or operating theaters, and environmental hazards are triggered by player actions such as failing a surgical quota or making too much noise. Mastering the patterns of each type is the key to efficient organ harvesting.

Roaming Entities: The Hallway Patrol

The most common enemies you will encounter are roaming entities that wander the hospital hallways. These include the Attendant, a tall, slow-moving figure that patrols the main corridors. It is drawn to sound, so walking instead of running is essential. The Attendant cannot enter rooms but will wait outside if it detects you inside. The Orderly, a faster, more erratic entity, breaks into rooms if you leave the door open. It is triggered by failed organ extractions and has a distinct wheezing sound. Finally, the Shadow is a rare entity that only appears when the hospital lights flicker. It moves through walls and can appear directly behind you. If you fail to meet your organ quota by a certain time, the hospital's Alarm System activates, summoning all roaming entities to your location for 60 seconds. Making excessive noise, such as dropping heavy equipment or using power tools, attracts the Attendant and Orderly to your current room. Leaving blood trails from failed surgeries attracts the Shadow to your path. The game also features Gas Leaks in the basement that cause hallucinations, making you see false enemy types. Checking your map and using the ventilation system to travel between floors can reduce your exposure to these hazards.

Elite Enemy: The Nightmare

The Nightmare is a rare elite enemy that only spawns after you have harvested five organs in a single night. It is a large, amorphous entity that blocks exits and can teleport short distances. The Nightmare cannot be outrun; you must use the hospital's Electromagnetic Pulse (EMP) devices, found in the security room, to temporarily stun it.
